Millennium High School is required to report total amounts for financial aid and scholarships that our students have received. Seniors must complete this form for EACH of the schools to which they have been accepted. Do NOT enter dollar signs with the dollar amount. Please complete the form below. All fields are mandatory – if the amount is zero, please enter 0. If you have been accepted to more than 12 schools, complete this form, submit the data and return to complete another form until you have entered the data from all of your schools.
